Places for People uses data, our emerging expertise with primary care medicine, and our understanding of the needs and challenges for people living with serious and persistent mental illness, substance use disorders, physical health needs and poverty, to support the individuals we serve. Our treatment teams practice integrated healthcare. Our Integrated approach will result in improved health outcomes and greater health literacy for our staff and the individuals and families we serve.

Overview

At Places for People, we treat the whole person.

Studies indicate that people with serious mental illnesses die an average of 25 years earlier than the general population. However, the causes of death are medical illnesses that were not treated or risk factors that could have been modified, including high blood pressure, diabetes, or substance use.

Places for People is committed to improving the health of the people served. The majority of persons served by PfP have at least one chronic physical health condition in addition to a serious mental illness or substance use disorder, and most also have significant histories of trauma that has been unidentified and, therefore, untreated.

Places for People is using our data, our emerging expertise with primary care medicine, and our understanding of the needs and challenges for people with serious and persistent mental illness to craft approaches that will result improved health outcomes and greater health literacy for our staff and our clients.

Partnerships and collaborations help Places for People support the individuals we serve in their healthcare journeys.

Health Home

Launched in 2012, our Health Care Home program is available for children and adults to improve health outcomes by coordinating physical health care.

Nurses for every team

Each treatment team at Places for People includes a nurse, or has access to a nurse.

Partnership with Family Care Health Centers

In 2015, Family Care Health Centers opened a satellite site with two exam rooms at Places for People, in a project supported by a grant from Missouri Foundation for Health. FCHC also provides support for a weekly “Team Care” meeting, which allows team members an opportunity to discuss questions and concerns in an in-depth fashion with a physician.

Partnership with West Pine Pharmacy

In 2013, West Pine Pharmacy opened a fully-operational pharmacy at Places for People, which provides Places for People staff the opportunity to consult daily with pharmacy, and allows opportunity for timely interventions.
